What is the Access 360 Portal?

The Access 360 Portal allows providers to manage AstraZeneca medications for patients online. Provider Portal offerings include:

  • Submit requests for patients such as Benefit Investigations, Pharmacy Submissions, and Electronic Prior Authorization submissions
  • Check status updates
  • Advanced features such as customizable alerts and multiple location access
  • Training from our Fielda Reimbursement Managers

What You Need to Register Your Practice

You will need the following information in order to register your practice:

1. User information including email addresses (user type, name, email address and password. You may also add additional users at a later date)

2. Practice location information (practice name, practice address, practice phone and fax number. You may also add additional locations at a later date)

3. Prescriber licensing information

  1. Prescriber National Provider Identifier (NPI) (required)
  2. You will be asked to agree to the Access 360 Portal terms. You must agree to these terms to proceed with enrollment on the Access 360 Portal.
